You must have your student athlete’s Full Name and Student ID number as they are listed in Infinite Campus, to begin the Electronic Participation forms.
If your student athlete is not listed in Rank One, please contact the site Athletic Facilitator for your school.
The OSSAA Physical Exam form is required to be completed and on file in Rank One, prior to any/all athletic participation at Moore Public Schools.
The form is two pages (Medical History and Physicians Exam page) and must be completed by an MD/DO, Nurse Practitioner or Physician’s Assistant, no earlier than May 1st of the school year prior to participation.
The OSSAA Physical Exam form must be the April 2026 version it is available to be completed online (Medical History and Athlete info on Physicians Exam page) and printed from the Download/Print Tab
The following Electronic Participation forms must be completed before your student athlete is permitted to participate in Moore Public Schools Athletics and Activities.

The OSSAA Eligibility Form and the Medical History Form will produce text boxes for all YES answers. Please explain all YES answers in detail.

You are not required to print the previously listed items. However, they are listed for you to reference at any time.
